Cognitive Assessment

An evaluation of memory, attention, language and thinking skills.

Overview

A cognitive assessment uses questions and simple tasks to check thinking skills such as memory, attention, language, orientation and problem-solving.

It helps to show whether there is a problem with thinking, how significant it is and which areas are affected.

Please confirm with the clinic when you book whether this assessment is currently available.

Why this test may be advised

  • Memory problems or forgetfulness that are getting worse
  • Confusion or difficulty managing everyday tasks
  • Concerns about dementia raised by the person or their family
  • Monitoring changes in thinking over time

Having one or more of these does not by itself mean you have a particular condition. A doctor's assessment is needed to find the cause.

What the test involves

  1. You will be asked questions and given short tasks, such as remembering words, drawing or naming objects.
  2. There are no trick questions; you simply do your best.
  3. A family member may be asked about changes in daily activities.

Who may need this test

  • Adults with memory or thinking difficulties
  • People whose families have noticed changes in memory or behaviour

How to prepare

It helps to bring:

  • Bring reading glasses and hearing aids if you use them
  • If possible, bring a family member or friend who knows about the changes
  • Try to be well rested on the day
  • Previous medical reports, prescriptions and hospital discharge summaries
  • MRI or CT scan films and reports, if you have them
  • A list of all the medicines you take, with their doses
  • A short note of your symptoms: when they started, how often they happen and what makes them better or worse

What happens next

  • The results are considered together with your history and examination.
  • Further tests, or a more detailed neuropsychological assessment, may be advised.
  • Repeat assessments may be suggested to monitor any changes.

Recommendations depend on the clinician's assessment of your individual situation. This website cannot diagnose your condition or guarantee a particular result.

Frequently asked questions

Can I fail a cognitive assessment?

It is not a pass-or-fail test. It helps the doctor understand your strengths and difficulties.

Does an abnormal result mean dementia?

Not necessarily. Anxiety, depression, poor sleep, illness and some medicines can all affect the results. The doctor interprets the results in context.
